Registration at WHO requires that you fill out basic demographic information about yourself (page 1), sign confirming you have received our privacy policies on page 6-8 of this packet (page 2), signatures allowing us to retrieve your medical information from your other physicians and health care team (pages 3 and 5), and sign that you understand our policy about how to handle complaints (page 4).
